Site 3 Collaborative for Emergency Care in Africa CECA 7 Background The USAF 59th Medical Wing Science and Technology Office has a requirement for research and development services This research study is titled An Assessment of an Accelerated Clinical Protocol for Rapid Return to Duty after Pneumothorax or Hemothorax ST1177 Return to Duty The approach to management of pneumothorax PTX and hemothorax HTX is complex and nuanced1 Tension PTX is the third leading cause of potentially preventable battlefield death HTX presents in onethird of thoracic injuries and is the fourth most common thoracic injury in modern combat 7 In the US civilian and military inpatient PTX andor HTX PTXHTX clinical management often lasts 137119days before the patient is discharged resulting in protracted morbidity2 In the deployed environment US service members with PTXHTX are often prioritized for evacuation and thus are unable to return to duty further straining combat capabilities3 In the future battlespace of antiaccess area denial and extended 72hrs evacuation to definitive care ie prolonged care settings an accelerated protocol to manage the safe return to duty of US military personnel with PTXHTX may conserve the fighting force and increase readiness where maximizing returntoduty is paramount34 An accelerated PTXHTX protocol may also reduce morbidity and health system costs for civilian populations across all income brackets All references can be found in the official contract file within the Statement of Objectives SOO and Statement of Work SOW documents   The intent for this study is to assess the safety and effectiveness of an existing accelerated nursedriven physiciansupervised clinical protocol that uses a dryseal intercostal drain system alleviating the need for external wall suction and emphasizes early mobilization This protocol is designed to promote ambulatory discharge within 72 hours in a realworld resourcelimited austere setting in the Western Cape of South Africa in which we have an established research network and infrastructure 8 Major Objectives   The main objectives are summarized as follows Define the demographics and injury characteristics of patients enrolled in the PTXHTX protocol who experienced the best outcomes from the protocol Evaluate relevant clinical endpoints of patients enrolled in the PTXHTX protocol time from injury and admission to discharge emergency department disposition intensive care and hospital length of stay final hospital disposition and mortality Establish the rates of adverse events noninfectious and infectious complications readmissions of the PTXHTX protocol 9 Information Requested This RFI is issued for informational and planning purposes only  Contact with Government personnel other than those
